Activity Guidelines and Forms
How to Get Started:
 

To submit an idea for a club activity, select the "Send an Email . . ." option below.  When your Email program opens, it will include the appropriate "send to" address for submitting information. 

You will add a Subject Line (example: "IL Club Activity Idea" or the name of the activity if already approved).

In the body of the Email, describe the activity and any details you have that are listed in the next section.

Type of Information to Provide:
 

The following information should be included if you are the activity chairperson and organization of the event is underway.  If you are submitting an idea, please include the type of activity and the name of someone who would be willing to be the organizer.

    • Event Name
    • Proposed Date (Month/Day)
    • Time
    • Where It Will Be Held
    • Cost Per Person
    • Deadline: (Reservations Due By)
    • Contact Person:  Name, Phone, Email (optional)
Send an Email to the Club President:                   
 

Select this Option to Initiate an Email to the IL Club President.  Be sure to include the details outlined above as well as any other information you want to share with the president.

Select this Option to Review or Print a copy of the IL Club Activity Guidelines.  These guidelines include information on chairperson responsibilty, notifications, paperwork required, financial responsibility and expense submition form, etc.
